Vacation Bible School

I/We, the undersigned parent(s) or guardian(s), hereby consent to my/our child,                                                                       , who is _____ years of age to participate in Vacation Bible School (hereafter known as “VBS”).  This activity is sponsored by Chapel Baptist Church (hereafter known as “the church”) from July 15-19, 2024. 

I/We certify that our/my child is able to participate in the activities associated with VBS unless otherwise indicated.  If my/our child has medical conditions that may be relevant to a physician in the event of an emergency, I/we have listed them below.  If I/we cannot be reached at the telephone number listed below, I/we hereby authorize the adult sponsor of this event,

Joseph P. Boots or his surrogate, to make emergency medical decisions for my/our child.

 I/We understand that all reasonable safety precautions will be taken at all times by the church and its agents during VBS.  I/We understand the possibility of unforeseen hazards and know the inherent possibility of risk.  I/We agree to hold the church, its leaders, volunteer staff, and employees harmless from any and all liability, actions, causes of actions, claims, expenses, losses, and damages on account of injury to my/our child or property, now and which may arise in the future in connection with VBS.

I/We understand and hereby agree to assume all risks that may be associated with said activity.

I/We understand that the church may record and/or reproduce on audio or video tape, photographs, or other media (including digital media) my/our child(ren)’s participation in VBS and I/we consent to both.  I/We agree that the church will be the sole owner of such recordings and can use them in any way it sees fit, without needing any further permission from me or my/our child.

I/We understand that the church will not release or publish my/our child’s identity in any of the recordings that are made.

I/We expressly agree that this release, waiver, and indemnity agreement is intended to be broad and as inclusive as permitted by the laws of the State of Florida and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.  This release contains the entire agreement between the parties hereto and the terms of this release are contractual and not a mere recital.

I/We further state that I/we HAVE CAREFULLLY READ THE FOREGOING RELEASE, KNOW THE CONTENTS THEREOF, AND I/WE SIGN THIS RELEASE AS my/our OWN FREE ACT.  This is a legally binding agreement that I/we have read and understand.
